Eligibility 

Individuals 16 years and older with mild developmental delay or dual diagnosis

Description of Service

Services include: time limited transitional supports (response time within 24 hours), onsite assessment and consultation and case management (1-6 weeks).

Counseling/Treatment: Individual, family and group; access to hospital supports, short-term case management, short-term and transitional support; after-hours crisis support.

Short-term Crisis Support: Residential support (0-30 days with possible extension) options for individuals who are not in acute psychiatric crisis and are able to be placed in the community.

Community Living Residential Option: for individuals who are homeless, at risk of being homeless, frequent users of hostel services, have had multiple hospitalizations and/or involvement in the criminal justice system.

Day Program: 30-90 day supports to transition individuals to less intensive community-based alternatives.
