Depends on service. Must have Ontario Health Insurance.

In-home health and social services: adults and children with any illness, physical, psychiatric or developmental disabilities whose needs cannot be met on an outpatient basis.

Placement in long term care facilities: adults 18 years and over with disabilities. Must need assistance with one or more of the followings: tasks of daily living, monitoring of medication availability of on-site nursing, or is at risk of being abused or abusing others, and community services are no longer sufficient.

Description of Service:

Arranges in-home or school-based health and social services, and placement in long-term care facilities, service coordination, information and referral for related community services.

In-Home Services: nursing care, physiotherapy, occupational therapy, speech-language pathology, social work, nutritional counselling, personal support, and homemaking. If eligible for above services, other services may be provided such as drug card, medical equipment or supplies, laboratory services, transportation.

Child and Family Services: for children or youth with short or long-term disabilities or other special needs and their families. Services available: in-home health and personal support (services as listed above), palliative care, respite care (caregiver relief), school-based services, professional services to enable children with disabilities or ongoing diseases to attend school.

Information and assistance: linking to existing community support services such as friendly visiting, meals on wheels, volunteer transportation security checks, attendant care for persons with physical disabilities and supportive housing services.

Placement in long-term care facilities: access for admission to a long term care facilities in Toronto or for transfer to facilities outside Toronto. Does not include senior's apartments, retirement homes, or chronic care hospitals.

Short stay care (vacation or respite): respite for caregivers, supportive care for those who need to recover strength, including persons with Alzheimer's Disease.

Crisis placements: when support suddenly breaks down at home, person is placed wherever a bed is available while awaiting placement in the facility of choice.

Mise en place à domicile ou en milieu scolaire de services sociaux et de santé, ainsi que placement dans des établissements de soins prolongés, coordination de services, information et aide à l’inscription pour d’autres services communautaires.

Services à domicile : soins infirmiers, physiothérapie, ergothérapie, orthophonie, travail social, conseils en nutrition, soutien personnel et tâches ménagères. Si vous êtes admissible pour les services ci-dessus, d’autres services peuvent être fournis comme une carte de médicaments, du matériel ou des fournitures médicales, des services de laboratoire, du transport.

Services pour les enfants et la famille : pour les enfants ou les jeunes handicapés à court ou à long terme ou qui ont d’autres besoins particuliers et à l’intention de leurs familles. Deux composants : 1. Soins de santé à domicile et soutien à la personne, services tels qu’énumérés ci-dessus, avec l’ajout de soins palliatifs et de soins de relève (relève pour les personnes soignantes) et de services en milieu scolaire; 2. Services professionnels pour permettre aux enfants handicapés ou avec des affections chroniques de fréquenter l’école.
