Description of Service:

Adolescent Inpatient Services: a 12-bed assessment, stabilization and treatment unit for patients who require specialized care in a secure setting

Eligibility

Adolescents 12 to 18 years of age who suffer from a serious mental illness or present with a complex clinical picture. Illness is often resistant to treatment and requires further diagnostic evaluation and treatment recommendations within a secure setting. Referrals are accepted by individuals, families, healthcare providers or social service organizations across Ontario.

Adolescent Outpatient Services: assessment, individual and group treatment for patients and their families, consultations with community agencies including school boards, and education initiatives with adolescent mental health care providers and other social service providers.

Eligibility: Adolescents 12 to 18 years of age who require individual, group or family therapy on an outpatient basis. Referrals are accepted by individuals, families, healthcare providers or social service organizations across Ontario.

Youth Case Management: assessment, treatment and life skills training for adolescents, as well as individual, family and group support.

Eligibility: adolescents 14 to 18 years of age who are awaiting treatment. Referrals are accepted by individuals, families, healthcare providers or social service organizations across Ontario.

 

Adolescent Day Treatment Program: provides mental health and education services in a local community high school to support students as they transition into a community setting. Services include individual rehabilitation therapy, life and social skills assessment and training, educational remediation and instruction, and recreational programming.

Eligibility:

Adolescents 14 to 18 years of age usually residing in the Region of Durham. Referrals are accepted by individuals, families, healthcare providers or social service organizations across Ontario.
